Transition of Care Materials Available for your Office

New resources have been developed to help transition the care of patients at 18 years of age from pediatric to adult medicine practices. These include a letter to the caregiver (Word doc) and a patient resource (PDF) that mirrors the other well child handouts that are available for your office. We welcome you to use either or both documents for your practice.

The letter offers variable fields and can be adjusted as needed. It aims to serve as a template for providers who would like to mail a letter notifying the patient’s parent/caregiver of the upcoming healthcare transition to adult medicine.

The “Your Healthcare at Age 18. How Will it Change?” patient resource aims to educate adolescent patients about the responsibilities of an adult patient. Ideally, this resource would be given a few years in advance of them turning 18 so they can begin thinking about the changes involved. This resource could be mailed with the letter and/or distributed in-person at an appointment. Providers could check-in with the patient at future appointments and address any questions or concerns. Parents could also use the resource to guide them on topics to further teach their teen about things such as refilling a prescription and making an appointment.
