Community Practice Support Tools – Check Them Out

Did you know that there are now eighteen Community Practice Support Tools?  These two-sided documents provide a concise, standardized review of common conditions. The information is presented as a one page written summary on the front and the same information as an algorithm on the back. All of the tools follow a similar format, providing a summary of the condition, suggested treatment, red flags, and clear guidance on when and how to refer.

You can find them on the Healthcare Provider resource section of the Cincinnati Children’s .org site. (Not CenterLink). https://www.cincinnatichildrens.org/professional/resources/community-practice-support-tools

They are also available in an app called “PCP Support Tools”.  The app sometimes lags behind the website, but will have quarterly updates.  If you have already downloaded the app, make sure that you have automatic downloads selected in your settings. This will ensure that you have the most recent app upload.  Questions?  Call your liaison for help.

Lastly, we will be providing the printed Community Practice Support Tools, in an organized binder for your use.  We hope to deliver these to your offices in August.  Thanks to all who have contributed to the creation of these, and we hope that you find them useful in your care of children.
