PATS E-Consult Improving Patient Care and Outcomes

Medication reactions significantly affect patient care and outcomes. To support our frontline providers, the Pediatric Antibiotic Allergy Testing Services (PATS) E-Consult tool was developed to ensure timely and appropriate care. This tool helps providers deliver optimal therapies, potentially saving families from unnecessary specialist visits and costs.

Utilization Options and Costs:

1. Penicillin/Amoxicillin Risk Stratification: Free

2. E-Consult: Approximately $150- Includes a written consultation with the provider and a nursing touchpoint with the family to discuss recommendations. ($)

3. E-Consult +: Approximately $150-$300- When ordering providers indicate that there is a current reaction, the E-Consult will automatically trigger a phone call to family from a specialized allergy nurse for triage. Families are offered a same-day/next-day visit (typically by Telemedicine) and are charged only for the service they utilize; E-consult or visit with drug allergy provider. ($ or $$)

Recent success story highlights from PATS E-Consults:

• 10-month-old with active rash: After starting amoxicillin for otitis media, a rash appeared. An E-consult indicating a current reaction led to a telemedicine visit, recommendations for treatment options appropriate treatment for the rash, and a additional follow-up call the next day. In the end, the patient was able to complete the amoxicillin course without further issues. ($$)

• 14-year-old female at a well-child visit: After noticing a remote history of amoxicillin allergy, the provider used the risk stratification tool within the E-Consult tool which indicated a calculated risk of “no risk to prescribe”. With shared decision/family consent, the allergy label was removed, ensuring future access to preferred antibiotics. (FREE)

• 6-year-old with SSLR: Presented to the ED with symptoms of a reaction after starting amoxicillin for pneumonia. An E-consult provided timely specialist advice to guide treatment($). Due to guidance provided by E-consult and confidence built with family This process allows for confidence to be built with the family, resulting in the patient returning to outpatient clinic for testing and cleared of amoxicillin allergy

• 10-year-old with rash history: Presented with otitis media. The risk stratification tool indicated no risk to prescribe amoxicillin, leading to de-labeling of the allergy. (FREE)

“[PATS E-consult] gives me a framework and the parents some reassurance to remove the allergy label. The families have all loved it because it does NOT require a visit or really any other time.  I don’t have any complaints. It makes it so much easier to have an updated allergy section. (the patient I saw today has been taking amox for years, but no one has removed the label, I don’t know why.) Thank you for developing it!” Dr. Alynna Schulert, Cincinnati Children’s Kenwood Primary Care
