Healthy Planet Dashboards – This month, HealthVine launched 3 additional Healthy Planet Dashboards with Northeast Pediatrics, Cincinnati Health Department and Primary Health Solutions, bringing our total to 5 Healthy Planet Dashboards! Before the end of 2021, all our partner practices will have a dashboard! Care Management Referrals - Patients with complex medical and social needs that are HealthVine members may be eligible for care management services provided by HealthVine and now providers can take advantage of a new order in Epic Link. Care Management Referral (CM123) can be found in the Order Entry activity by typing “care management” in the procedure search box. A job aide is available in Epic Link Quick Links. HealthVine PCP and Attribution Update: HealthVine has been working with CareSource to update the file that feeds the “HealthVine PCP.” In October, providers should see updates to patients that are incorrectly attributed. In addition, we are launching an Epic Link messaging option for incorrect HealthVine PCPs. Please send a message to the HealthVine PCP Change Pool (type in “p Healthvine pcp change” to populate the pool) in Epic Link to let us know of any errors.
