The GET2HOME Research Study Coming Soon; Pilot Study Going on Now

The GET2HOME Research Study is a collaboration between hospital medicine and primary care designed to improve hospital to home transition in children with complex chronic disease. The study is led by pediatric hospitalists Dr. Patrick Brady and Dr. Katherine Auger, was recently funded by the Patient Centered Outcomes Research Institute (PCORI) and is set to begin in November 2022. The study will be a randomized control trial of 600 total complex patients over 2 years, where half the patients will receive routine care and the other half (300 patients) will receive a pre- and post-discharge telehealth huddle (video visit) that includes the patient/family, the inpatient team, the patient’s primary care provider (PCP), and any other team members (care managers, social workers, pharmacists, subspecialists, etc.) that are able to participate.

To ensure that we are ready to go in November and have worked through any potential challenges, we have been (and will continue to) pilot test these huddles with ~5 patients/week who are being discharged from CCHMC’s Hospital Medicine Service. Early feedback from families and PCPs has been very positive—families feel better prepared for discharge and are able to get early post-discharge questions answered; PCPs have said that they have found these discussions collaborative and that they felt better prepared to coordinate care after discharge, including getting assistance with supplies and follow-up planning. The family or PCP is always able to decline participation, and we ask everyone to complete a REDCap survey to provide their feedback so we can improve the huddles. CCHMC is not billing the family for these visits.
