New Mood Action Plan Tool; Your Feedback is Requested.

We are excited to share with you a new tool for mood disorders called the Mood Action Plan.

The Mood Action Plan defines green, yellow, and red days depending on one’s symptoms and allows for customizable, planned “actions” to take in each scenario. Patients (often adolescents) can use it to know when to seek help, to pay attention to their symptoms, and to have access to important phone numbers. There are also FREE apps on the back page that patients can download for different symptoms such as “poor sleep” or “self-harm.”

This tool evolved out of quality improvement work, and we wanted to share it with the community. We have found it to be most effective for patients who do not want to take medication, who decline therapy, or who come back for a follow-up appointment after an ER visit or crisis. It is meant to be filled out with the patient. The medical provider can be anyone who treats mental health issues—doctors, NPs, social workers, psychologists—whoever feels most comfortable using it.

If you would like copies of the Mood Action Plan (electronic or tear off pads available) please contact your Physician Liaison.

Request for Survey Participation

Once you review this new tool, please consider filling out a brief survey available HERE. The survey should take <5 minutes to complete.

Your participation is completely optional, and having access to the tool is not dependent on completing the survey. We are interested in seeing how the tool functions in other clinic environments. It may lead to a research project in the future. If that does take shape, your responses will be aggregated, and your individual answers will never be used in a manner that would allow you or your practice to be identified (either directly or indirectly).

Thank you for your help with this project. We hope you find this tool useful. Reach out to your physician liaison if you have any questions or email Landon Krantz (landon.krantz@cchmc.org). The CCHMC Gen Peds Depression Team - Landon Krantz, MD, Rachel Herbst, PhD, Jennifer Hardie, MD, Mary Carol Burkhardt, MD MHA - appreciates your input.
