Hello to my fellow PCPs,
Thanks for taking the time to read Synapse. Inside, you will find great articles about the new partnership with UC to help prevent gun violence, updates on CME, and a reminder of all of the educational resources available to improve awareness about Lyme Disease. If you have not had the chance yet, the Pediatric Consult podcast on Lyme Disease featuring Drs. Liz Schlaudecker and Jillian Schaffeld is a helpful review of this disease that has become a lot more common in our region. I also learned a lot from the 2-part dental series by Dr. Paul Bunch and CCHMC Dental faculty Dr. Scott Schwartz. CPAC – Our May CPAC meeting was a discussion on care transitions from pediatric to adult providers. We had an excellent panel including Drs. Sarah Corathers (Endocrinologist and Ambulatory Chief of Staff), Nicole Brown (Cardiologist and Medical Director of Cincinnati Children’s Adult Care Services), and Emily Johnson, CPAC member and faculty in the Freeman Center at UC. As we learned, intentionally preparing our older patients for this change can truly make a difference in their ability to successfully establish regular adult care. Simultaneously our POE group has been working on templated resources which practices can use to help guide their older patients in the process of transitioning to adult care, which will be shared in the June issue of Synapse and distributed directly to your practice by your physician liaison. If you are looking for resources, Got Transition (www.gottransition.org) describes the Six Core Elements of Health Care Transition adopted by the AAP, AAFP, and ACP. The CCHMC ambulatory team has begun the discussion of how to work on this for specialty services as well.
DDBP Update – Many thanks to our colleagues in DDBP who have worked diligently to offer autism diagnostic training to regional PCPs using the ECHO system. The initial cohort will begin in October 2024 and this first group is full! I am so thrilled with this partnership, and the potential to help children with more accessible and earlier diagnosis of ASD.
Asthma Care – Please keep an eye out for the information from Therese Martin regarding the expansion of Remote Patient Monitoring for children following discharge from hospitalization for asthma at Cincinnati Children’s. For the last couple of years, Cincinnati Children’s has employed RPM for monitoring children who are their primary care patients after an asthma hospitalization. RPM has been successful in this population and Cincinnati Children’s is planning to expand this to ANY child hospitalized for asthma in the near future.
E- Consult – Please also look for information in June regarding changes coming to E-Consult. As many of you may know, “E-Consult” is an Epic functionality that Cincinnati Children’s has made available for PCP connections regarding new patients. As Cincinnati Children’s has worked to improve access at the institution, they have identified opportunities to develop a more detailed encounter through E-Consult for certain conditions with longer wait times. More information will follow from Therese soon.
PCP Survey – Thanks to all who completed the PCP survey. CPAC and Physician Outreach work together to review your input, which helps us identify where the opportunities are to strengthen the interface between the hospital and community PCPs. If you have not had a chance to complete it, please do! A reminder will be sent by Therese Martin on Thursday morning, May 30.
