Epic Link Tip: Psychosocial Health Report

In May’s edition of Synapse we shared about the SDOH screening and the SDOH wheel available in EPIC link.  We also wanted to share about an additional resource.  As part of the institutional taskforce to make recommendations on an approach to screening and responding to 4 domains of psychosocial health (social determinants, depression, suicide, and substance use), a report called the psychosocial health report was created to allow providers to see results of these screeners wherever they occurred across the care continuum at CCHMC.  In addition, you can access the patient’s safety plan if they have one through this report as well. 

This psychosocial health report is available to community providers through EPIC link using the instructions found HERE.   You can see the last 3 results/dates of the screeners for each domain.  Currently only certain areas of the hospital have been testing the implementation of these screeners  as part of the taskforce, but include general and community pediatrics, adolescent medicine, Endocrine-diabetes, partial hospitalization, and CHECK clinic.  Sickle cell, Complex Care clinic, A6NS, LA-4, Peri-op consult, and a small number of surgeons in orthopaedics for certain clinical visits implemented screening for SDOH only. If other areas in the hospital were using depression screening using the PHQ-9 prior to the taskforce those results will also populate the psychosocial health report.
