Care2, The Ohio AAP Maternal and Child Health Project, is Enrolling Now

The Care2 Project helps maximize time in pediatric visits to address the health needs of mothers and young children at the same time. Many mothers take responsibility for attending their children’s appointments – but do not regularly receive care for their own health. Care2 is working with practices to address this gap and integrate two generation care into existing workflows.

Utilizing common practices for addressing smoke exposure, maternal depression, and family planning as part of a quality improvement collaborative, the Care2 project was designed to support physicians and other healthcare providers in practices that see both mothers and children under 5. Ideal participants may include:

  • Family medicine or family practice locations

  • Internal medicine/pediatrics locations

  • Federally Qualified Health Centers (FQHCs)

  • Health departments

We are currently enrolling practices to engage in Wave 2 of this project from May 2024 – May 2025! Participants in Care2 will receive:

  • A $3,000 stipend to support data collection

  • Free sleep sacks and board books to promote safe sleep with your patients – up to $2,000 value

  • Maintenance of Certification (MOC) credit upon completion of QI activities

  • Support from a dedicated Practice Coach

  • Learning collaborative to share challenges and success with peers across Ohio
